How can I make a claim?

1. Our support staff can assist with in processing your claims​.​

  • For motor insurance claims, contact us immediately at 02-202-9500 and our staff can support you with incident inspection.
  • For other claims, contact 02-202-9500

2. We will directly contact you within 3 days if any additional information is required.​

3. Claim processing can take up to 7 working days and we will provide an update on the status of your claim before the end of this period.

Note: Proof of identity is required for insured, beneficiary or the person who has the right to claim in order to process claim (in compliance with The Anti-Money Laundering Act 1999).​

How to file a claim?

1. Download and complete the form for your product from the list below.

2. Prepare the required documents stated in the form​.

3. Submit claims form and any required documents​.

When will I get my claim?

1. Motor Insurance (CTPL):

  • Initial medical payment within 7 days from the date we receive the complete set of required documents.​
  • Excess damages claims within 7 days from the date the compensation is agreed and we receive the complete set of required documents.​

2. For all other products:

  • Claims will be paid within 15 days from the date the compensation is agreed and we receive the complete set of required documents.​

Claims forms

Claim form for Personal Accident policy (To be filled by individual)

Claim form to be submitted for individual or group policies, for coverage of medical expenses, compensation for death,
disability or dismemberment.

Claim form for Group Student Personal Accident Policy (To be filled by Student or educational institution)

Claim form to be submitted for coverage of medical expenses, compensation for death, disability or dismemberment.

Consolidated student claims form – Group Student Personal Accident Policy
(To be filled by educational institution)

Claim form which can be used by educational institutions to consolidate the names and claims from of multiple students.

Hospital Transfer form for Student (Accident)

Form used by educational institutions on behalf of students when a or student needs to be transferred to a specific hospital
to receive treatment.

Add beneficiary form

Form used for adding a beneficiary if the insured has passed away without designating beneficiary.
To be filled by insured's family.

Letter of guarantee and waiver (Individual)

Letter of guarantee (Page 1):
In case the insured dies without designating a beneficiary, then someone from the family should complete this form declaring
themselves as a beneficiary to receive the compensation. This letter is a guarantee that you are the beneficiary and if you are
filing the information correctly, otherwise you can be fined/prosecuted. The form also needs to be co-signed by a guarantor.

Waiver (Page 2):
If there are multiple beneficiaries and they want the entire compensation to be paid to 1 single beneficiary instead, then all
the beneficiaries need to complete this form.

Medical and Accident claim form for Covid-19 policy (Individual)

Form used for compensation claims due to COVID-19 infection including compensation for death,
disability and dismemberment.
